How long does orthodontic treatment actually take? The honest answer.
It’s one of the first questions almost everyone asks at a consultation: how long is this going to take?
And we get it. Before you commit to braces or Invisalign, you want to know what you’re signing up for. Will it be six months? Two years? Somewhere in between? The answer — and we’ll give it to you straight — is that it depends. But it doesn’t depend on random factors. It depends on specific, predictable things. And once you understand them, you can get a very realistic picture of your own timeline.
Here’s the honest breakdown.
The typical range for most patients
For most children and teens, orthodontic treatment takes between 18 months and 24 months. That’s the most common window — not a worst-case scenario, not a best-case scenario. Just the realistic average for moderate cases involving crowding, spacing, or mild bite issues.
Simpler cases — minor crowding, small gaps, minor alignment issues — can sometimes be completed in 12 to 18 months. More complex cases involving significant bite correction, severe crowding, or jaw development issues can extend to 30 months or more.
Adult treatment timelines are similar, though adult teeth tend to move slightly more slowly than younger teeth. Most adult cases fall in the 18 to 26 month range.
What makes treatment faster or slower?
The complexity of the case is the biggest factor — and it’s the one neither you nor your orthodontist can change. More movement required means more time needed.
But there are factors within your control that have a real impact:
Compliance: For Invisalign patients, wearing aligners 20 to 22 hours per day is non-negotiable. Patients who consistently skip hours add weeks (and sometimes months) to their treatment. This is the number one reason Invisalign treatment runs long.
Keeping appointments: Every check-up appointment allows us to make adjustments that keep treatment on schedule. Missed or frequently rescheduled appointments mean lost time that’s hard to make up.
Oral hygiene: Poor hygiene can cause gum issues that slow treatment or require us to pause. Keeping your teeth clean is genuinely part of keeping treatment on track.
Wearing your retainer: After active treatment ends, a retainer holds your teeth in place. Patients who skip their retainer often see shifting — which can mean additional treatment.
What about those '6-month' treatment options you see advertised?
Accelerated orthodontic options do exist, but they come with important limitations. They typically address cosmetic alignment of the front teeth only — not bite correction, not back teeth, and not underlying structural issues. They may be appropriate for very specific, simple adult cases.
We’ll always be upfront with you about whether a short-term option is genuinely right for your situation — and when it’s not, we’ll tell you why.
Can treatment be sped up?
To a degree, yes. There are FDA-cleared devices that use gentle vibration or light force to help teeth move slightly faster. We can talk through whether those options make sense for your case.
The most reliable way to stay on schedule, though, is simple: wear your aligners, keep your appointments, brush your teeth, and follow your care instructions. Patients who do all of those things consistently almost always finish on time or close to it.
Common questions about treatment timelines
Can you give me an exact finish date?
We can give you a strong estimate after your initial records and exam — but not a guarantee. Treatment timelines can shift slightly as teeth respond to movement. We track progress at every appointment and will always keep you updated.
What happens if treatment takes longer than expected?
It happens sometimes, and we’ll communicate with you early if we see the timeline shifting. Additional refinements (especially with Invisalign) are sometimes needed near the end of treatment and are included in your treatment fee.
Does it take longer for adults?
Slightly, on average. Adult bone is denser, which means teeth move a little more slowly. But the difference is usually measured in months, not years — and many adults are pleasantly surprised by how quickly treatment progresses.
